Social Prescribers
Our Social Prescribing Link Workers give our patients the time to focus on ‘what matters to you’ and take a holistic approach to your health and wellbeing.
Your conversation can begin with either a referral from your GP, nurse or member of our practice team, or you can self-refer yourself for an appointment.
The appointments with a Social Prescriber can be carried out on the telephone or face to face and are usually longer than a standard GP appointment.
Our team are here to listen and connect patients to local community groups and statutory services for practical and emotional support
